Charlton Aged Care

Charlton Aged Care in central Charlton is a 13-bed aged care facility providing friendly, comfortable and personalised care to everyone who lives there.

What we offer

Each room is furnished with a chair, table, bed and fittings and residents are encouraged to decorate their rooms with belongings and mementos to make it feel more like home.

Our home is staffed by highly skilled professionals, supported by a large range of co-located services to ensure all needs are met. Our small homes ensure that our team knows the needs and preferences of all of our residents and we maintain strong relationships with friends and family. Our staff know you and you know them. We work in accordance with or core values of honesty, empathy and respect.

Privacy, freedom of choice and shared decision making are key elements of our approach to care, and we aim to support each person to live the best life they can. We promote the interests of each resident, and everyone is encouraged to have input into our lifestyle program with activities then tailored to reflect the preferences.

We actively promote meaningful partnerships with our families and friends, believing that by everyone contributing, everyone belongs. We host informal meetings/events, where resident’s families are invited to come along and share a meal, some games and friendly conversation.

We celebrate milestones and stay abreast of current affairs and public events.

Our philosophy is to ensure and enable all residents to live their life with dignity and comfort whilst encouraging them to pursue their own concept of a joyful and fulfilling life.

How to access

You will need to undergo an assessment with the Australian Government’s My Aged Care service. Our Campus Manager can help you through the process and answer any questions you may have in relation to accessing our aged care services.

Cost

What you pay to enter care depends on your income and assets and is determined by the Australian Government. You can find out more about rooms and costings at Charlton Aged Care on the Australian Government’s My Aged Care website.
